About the Role
Supply Hive are working alongside a specialist education setting in High Wycombe who are looking for a dedicated Transition Support Tutor to join their team.
This is an exciting opportunity to support young people aged 15–25 who may need a more personalised route into education, employment and adult life. Many of the learners have SEND and SEMH needs and may have experienced anxiety, trauma or barriers to engaging with traditional education.
This is not a traditional classroom-based role. You may spend part of your week visiting learners at home, meeting them in the community, supporting them to develop life skills and confidence, or helping them take the next step towards education, employment or greater independence.

The role will involve:
- Delivering one-to-one and small-group sessions focused on life skills, confidence, wellbeing and individual goals.
- Building positive relationships with young people who may be reluctant to engage.
- Supporting learners with SEND and SEMH needs using a calm, trauma-informed approach.
- Helping young people develop confidence, resilience and independence.
- Supporting learners through their transition journey, including home-based support, community activities and centre-based learning.
- Planning activities around individual interests, needs and EHCP outcomes.
- Supporting life skills, employability, English and Maths where appropriate.
- Maintaining accurate records of learner progress, engagement and wellbeing.
- Working closely with families, carers, colleagues and external professionals.
- A full UK driving licence and access to a business-insured vehicle.
- Experience supporting young people with SEND and/or SEMH needs.
- Experience in youth work, mentoring, social care, education, family support, coaching, employability or a similar setting.
- The ability to build meaningful relationships with young people who may be reluctant to engage.
- A calm, patient and consistent approach.
- Strong communication and interpersonal skills.
- The ability to work independently, including occasional home visits and lone working.
- Good organisation and record-keeping skills.
- A genuine belief in the potential of young people who may have struggled in traditional education.

- Be empathetic, patient and open-minded.
- Be willing to adapt their approach to meet individual needs.
- Support rather than direct.
- Be passionate about helping young people become more confident and independent.
